Every year when Washington Youth Soccer families pay their club fees, a small amount of that fee goes towards "insurance," a benefit of being a member of Washington Youth Soccer.
What is insurance?
Insurance provides Washington Youth Soccer Associations and Clubs with necessary coverage to allow them to book games and practices at public parks and fields, as well as medical insurance for both players and coaches.
Who does it benefit?
In the event a player is injured during a Washington Youth Soccer-sanctioned event, excess accident medical coverage helps families pay related costs above those covered by their primary insurance provider, less a deductible. If the family does not have primary insurance, then their excess accident coverage would act as primary insurance, helping families through what could otherwise be a costly ordeal.
How does it work?
Follow the steps below to complete your Medical Insurance Form:
Step 1
Your player (or a coach) is injured. Seek appropriate medical help immediately.
Step 2
Download the "2011-2012 Accident Medical Claim Form"
Step 3
After completing the form, make sure that it is signed by your coach before it is submitted.
Step 4
Submit the completed form to Washington Youth Soccer, either by scanning and e-mailing to Jillian Higgins at jillian@WashingtonYouthSoccer.org, or by faxing to 253-925-1830, or mailing to Attn: Jillian Higgins 500 S 336th St. Suite #100 Federal Way, WA 98003.
Step 5
Washington Youth Soccer will verify that the information on the form is accurate and that the injury indeed occurred during a Washington Youth Soccer-sanctioned event. Upon verification, the form will then be submitted by the state office to Pullen Insurance.
Step 6
Upon receiving the form, Pullen Insurance will contact your primary insurance provider to determine what portion of your costs are already being covered. Pullen will then follow-up with you directly to coordinate your additional accident medical coverage.
