WSYSA provides Certificate of Insurance for soccer fields in a variety of locations, including those owned by schools, parks, and private entities. Certificates of Insurance are issued to Associations not Clubs.

Complete the form and then click save. The application will then be submitted electronically to WSYSA. A Certificate will be mailed to you within 10 days.
